A <strong><em>Request For Assistance Form With Physician Prescribed Medicine Waiver Of Claims And Release Of Liability\u00a0<\/em><\/strong>form must be filled out prior to medication being administered.<\/span><\/h3>\n<h3><span style=\"color: #ffffff;\">For non-prescription medication or <em>over-the-counter<\/em> medicine,\u00a0<\/span><span style=\"color: #ffffff;\"><strong><em>Request For Assistance Form With NON-Prescribed Medicine Or Over-The-Counter Waiver Of Claims And Release Of Liability<\/em><\/strong>\u00a0must to be filled out prior to medication being administered. It is the school's preference that medication be scheduled outside of school hours whenever possible.
